I find the start of the game exhilarating, specially with small republics. However, mid and end game is more tedious.

What about crisis that you cannot avoid by good management. I mean, civil wars and rebellions are crisis that you should be able to manage. What I am thinking is a Stellaris Mid or End game type of crisis. You cannot prevent them nor you can blame yourself, but they make your empire survival exciting again. These have to be related to historical events: volcanos, illness, invasions, etc...

Others before me have suggested this (@EricMN93 on Jan21,2020 post Final Crisis) but I am not sure if this topic has been brought to the Senatus Populusque.

honestly there are quite a we could get more challanges lategame. Some ideas could be:

As you said a disease/plague system maybe look at ck2's reapers due?

Expand upon religions again in a future patch, non state religions cannot spread atm (exept with events) which i really feel could be expanded on in the future. Maybe some religious decitions to repress other religions and whatnot. (buddhism/kemetic currently got some special events to spread but it feels strange/bad that it has to be driven by events)

Make barbarians a threat to civilized countries. Stuff like the cimbrian war would not be a threat to rome as the mechanics currently are.

have some modifier where territories far away get a -% modifier on output based on their distance from the capital? would be reduced by roads/sea acess and maybe have some way to designate metropolises to radiate controll aswell? (think antioch/trier for rome) Brittain was basically a gold sink for rome for example it cost them more to hold the province than they got out from it.

Make standing armies behave as mini countries of their own. This would be hard to do right but it's not enough that a army got a loyal commander the army itself should be able to mutiny and declare their own ruler if they are unhappy due to low pay or simular. (think the many soldier emperors of rome)
