Some general ideas regarding vanilla features that probably shouldn't exist in the mod:
1) Peasant revolts; in game, it's not uncommon to see the Great Houses of Westeros lose their hereditary seats to random peasant uprisings. Peasant revolts causing independence should probably get chopped from the mod completely (it doesn't fit the lore).
2) Merchant republic CBs; the 'take city/coast' CBs should be removed (otherwise you get weird situations like Braavos conquering Driftmark).
3) Claim fabrication; the ability to fabricate claims should be severely constrained (e.g. republics should not be allowed to fabricate or be targeted).
4) Random landless raider bands; the system of random Dothraki or Ironborn characters spawning ~5k troops from thin air and randomly raiding provinces needs to be chopped.
5) Independence factions; my main problem here is with Riverlands lords declaring independence from the Ironborn in the Bleeding Years scenario. While they should be opposed to Ironborn rule, the problem with the current setup is that it results in a mess of independent duchies- they should be more likely to support a local claimant to the Kingdom of the Trident rather than becoming independent outright.
