mod custom holy order

the problem i'm facing is about modding, i add custom decision to add custom holy order. the decision is exist there, the reduction of money and piety also work. the character exist, but it didn't show at holy order table, he just a normal landless someone with duke tier title, how i can solve this? also if possible how to erase the decision after it so the holy order can only be made one?
