My prisoner is in a county far, far away

Description​

As Duke of Bohemia, I imprisoned my cousin after he revolted, refusing to give me his land I rightfuly owned (how dare he). He soon became my rival, so I let him rot in jail. But a few years later, as I wanted to tort... *cough* *cough* errr... pay him a visit in his jail, I noticed that, somehow, my rival was in "my prison"........... in "Navarra" (wat ?!).
Consequence is that I can't interact with my prisoner anymore, being outside of Diplomatic Range.

Steps to reproduce​

Unfortunately, I don't really know. Imprison someone, and wait for a few years ?
I also noticed that his wife and two kids were wandering quite far away - is there a mechanic so that families try to stick somewhat together (even if it means a prison teleporting WITH his jail ) ?
