My situation was that my current character had two ducal titles (Munster and Meath). He was also incapable and under his wife's regency.
His only child, a son, had died earlier but his father had sired many other sons. The succession law of the duchies was primogeniture, so the oldest living brother that wasn't a bishop was flagged as heir.
This character had been named Doge of Ormond earlier on and I had given him the whole Duchy of Leinster some time ago, but his title remained Doge.
When I died... *poof* Game Over. I can only assume that this was because even after inheriting two more Duchies, the man still kept his Doge title as principal title. And you're not allowed to play as a Doge...
But this was never announced to me. I was playing along, just one blink away from Game Over and I didn't get a single warning! I could have revoked his Doge title, he'd have been pissed but he'd have kept his Duchy and he'd have inherited it all without a sweat. But since I never got any advance warning...
