I enjoyed the game, and that you could target enemy unit stacks in regions(unlike Making History II- but they reduced any city IC to '1' from a nuke attack), but my game(HOI2D) showed the target's IC to be undamaged(no reduction/penalty). I liked the little mushroom cloud graphics effect.
The only 'odd' thing I experienced in the game, was near the end. In the Pacific(I was playing as Germany), the USA had over 100 Divs on an island-you could not see the island it was so small, only part of the Port icon. I nuked it-to help my Japan Ally- and caused huge troop losses to the USA(repeated this elsewhere too-nuclear surgery-good for altering strategic outlooks).
