The way I did it was: Mendog as king, in mid '37 there's an event proposing rebirth of the Commonwealth, you don't have cores on Galicia/Wielkopolska, then you choose government, army, ruling party, and then there's some messy stuff about Dmowski and you switch back into Poland ;P.

Poland annexes Lithuanian and then changes tag from POL to (IIRC) U37. So, yeah, name and flag changes.
