You have an nVidia problem:

That can be driver or card! Try this:

- uninstall that driver https://developer.nvidia.com/cleanup-tool
- reboot
- install the latest by fresh download https://www.nvidia.com/Download/index.aspx?lang=en-us
- reboot again

Are you still getting that nVidia error in dxdiag? If so, I fear there's a hardware problem there!
