How do you remove counties/provinces from kingdoms and duchies or transfer them to another kingdom/duchy?

Btw. I mean the prerequisites for creating a kingdom/duchy not removing county from kingdoms/duchy control.

edit the province.csv, find the county you wish to move to other entity, each county may belong up to two entities, two duchies, two kingdoms, a duchy and a kingdom. it is irrelevant if you put the tag in either duchy or kingdom column. NONE serves as filler when you don't need the county as requirement to a title.
