I'm playing England in the GC and I'm in war with Scotland for the second time, the first time I captured the 2 northernmost provinces, so the only remaining Scottish ones was Strathclyde and Lothien. In this second war I'm controlling the two last provinces (Strathclyde and Lothien) and I've beaten all the armies and fleets that I can see (Scotland is allied with France and was together with France in a war with Spain, so he might have sent something south before I attacked).

The problem is that if I go to the peace screen I can demand money and Strathclyde but I don't get any option to annex them, which after reading on this board I beleive I should ? I've only got 6 stars if that helps ? In that case how do I get the last 7th star ?

You must have control over all the provinces of your intended new 'ally and friend'. This includes the provinces they might have taken over-seas (colonies; enemy territory under their control). It also apparently includes tradeposts, which is a bit of a hazzle, since especially the colonizing nations keep churning them out, which makes it nigh-on impossible to annex them...

Also, something to remember is if they think their REAL friends might come beat you up, they could also refuse an annexation, but after proper 'consideration' they usually give up.

Since this is Scotland I don't think they got any Colonies or trade posts but I can of course look...
But your second point is valid if not a bit annoying.... I've got the clearly largest fleet of me and France by about a factor of 2, and mostly all of it is currently sitting in the channel to stop any french invasion, but France got more troops than me, so I if the Scottish AI sees France as stronger than me, I can never annex them as long as their allied to France!!
And then how do I break that alliance ?

Actually, if you don't even get the option to annex, scotland is still in control of some piece of territory you A) Don't know about or B) Have overlooked.

Only when you've given them your peaceproposition 'Awrite ya scottish bastiges, give up or die!' can they refuse (LEHNTE AN, the words we love to hate) 'Shut ye face, foor ai kick yoo in the noots' and in that case, you can sue for peace with France first, and once France is out of the war, no one will help poor Scotland.
 
There may be more to this...

In a recent game I captured all Hansa League provinces. I'm absolutely certain, since the peace negotiation screen told me so, and I had a zillion stars. But I was still unable to annex them.

Yesterday my allies and I captured all Genoa provinces, but in my peace negotiations I was unable to demand any provinces.

Is there a bug here, or am I missing something? I play the German version with the 1.03 patch.

When starting a new game (and only then) you are given the choice to play with forced annexation ON (ein) or OFF (aus).

My first game I inadvertadly started with this option off (I think that is the default) only to find out (in about 1650) when I tried to annex a country that I knew did not have any colonies. Previously I had assumed that the country I tried to annex still had some unidentified colonies (maybe in terra incognita). So there...

Nope. That's not it. I got annexation ON. Appearantly that's the default.
