So I'm playing a mulitplayer game with a couple of friends and we've incorporated our own homemade COAs for our dynasties into the game, which was simple enough. And because we want to see those COAs show up as much as possible we want to have them replace (or be incorporated into) the kingdoms we create and control. So, we've done this before and it's simply a matter of putting an a replace image for the flag in question into gfx/flags in the mod folder, for instance replacing putting in a new k_england. This has worked in the past. However for some reason now, it's not working and I honestly can't figure out why.

Anyone know? Has anything changed? I hadn't really played since 3.0 prior to this.

Have you tried clearing your flags cache in your CK2 folder in your My Docs?
