Please explain your issue is in as much detail as possible.
When creating a custom empire ruler titles are only generated once. This has two effects:
  1. With sex specific titles, such as "King" and "Queen", only the titles for the originally selected sex will be correct. When switching to the other sex, the ruler title remains unchanged
  2. When changing government type after customizing your ruler, the titles don't update either, so you can have an empress lead a democracy without changing the titles manually

Steps to reproduce the issue.
  1. Create a custom empire
  2. Select imperial government
  3. Go to ruler section
  4. Change the rulers sex
  5. Bonus: Change government to democracy

The title should remain as "Emperor" or "Empress" depending on whichever sex was pre-selected and stay that way despite changing sex or gov type.

Any update on this one? I've got the same issue.

I believe the fix is intended for 3.4.4.
There is a workaround until then:
You can still have different titles if you first set the title for one, then the other and then start the game without going back to the first. The title for one gets overwritten when you change the gender, not when you set the title for the other.
