Adoption IAM adds a decision to adopt a character. Very uncommon and ahistorical, but that is the point.
The adopted character has to be a ♥♥♥♥♥♥♥ or lowborn, younger than 21, landless and unmarried. They also cannot be imprisoned or in hiding.

You have to be older than 42, not in hiding and not imprisoned. It will cost you 100 prestige and 100 piety.
However, this will make your entire family and vassals hate you (-50 opinion).
Your landed family members will hate you twice as much.
So, you really want to use this if you are very popular, very powerful, or in a small realm.

For roman culture there is a different kind of adoption.
This kind of adoption is intended to create alliances. You can adopt children of other characters who are not heirs. The character you adopt from will become your friend, i.e. an ally. Their title tier has to be the same as yours or lower, and they need to have a very positive opinion of you (75).

I am not part of the CK2+ team, so if you use this mod your checksum will change and your CK2+ warranty will be void. But hey, no pain, no gain.

Thank you for this. I'll probably change it so that there is no age requirement on the performing character's end, and that the adopted character is hated by your family/vassals.

This will allow me to make a choice in waiting for an heir or adopting someone without it being too cheesy.

I don't know how the game would react if a "child" is older than "parent".
I was thinking of adding ability to adopt someone as a sibling, which will include them in the dynasty but without inheritence. And then i could add a second decision that would make them eligible for inheritence.
 
This could be adapted to be more historical if only Republics and the Byzantine Empire could do it, via the Designated Heir function. Bloodlines didn't matter so much in Republics, and adopted heirs were relatively common in the Roman Empire.
