Take over a country by force?

I'm polying a campaign (don't remember which one) and I've completely crushed another country and taken over all of its provinces, including the capitol, how do I conquer the entire nation?
The capitol can't be demanded in a peace agreement...

Which nation are you trying to annex?

Major (e.g. playable) nations can't be annexed, but the save file can be altered so that they aren't major nations any more. If it's a minor nation, then you should have the option of annexing them. If you don't, then you must have missed a province somewhere.

And are you sure you got them all? Maybe there is a tradepost or colony hidden somewhere in the world?

And have you turned the forced annexation option on?
