Terrible framerate despite solid PC and low GPU+CPU utilisation. Also Vsync displays some fascinating behaviour.

Even more curiously after doing some tinkering I found that setting Vsync to "quarter" capped my framerate at 15fps. Which is very weird as I have a 165hz monitor! The game appears to think I have a 60hz one.

Do you have a FPS Cap active in the Driver Software somewhere? Is the monitor plugged into the GPU port on your PC?
