So, how does this decision work, as the title says?

Does the Schism still happen after the millennium if you do not take this realm decision? If you take this decision, does the schism happen sooner? Can someone please explain? Thanks kindly!

The decision is there if you want to start the game with the Schism already in place (Chalcedonian Christianity split into Catholic and Orthodox). Some folks whine about Chalcedonian as a concept, so it's there as an option.

If you play, the Schism can happen on its own (and in the 1000 bookmark, is more likely to since some things which cause it have already occurred).

Well yes, and no...

Sometimes the Bulgar Slayer goes apeshit, takes jerusalem, and appoints patriarch. Seen it happen a couple of times.
