I have prisoners who are unable to be assigned to a cell. I have a sufficient number of unallocated regular cells and padded cells with "shared" access. Gangs are disabled in this game, so they are not a factor.

My current working theory is that the prisoners who have this problem are ones who have obtained the "criminally insane" attribute by virtue of being identity thieves. The problem seems to come up most often when it is time to release one of these prisoners from solitary confinement.

Frequently (but not always), I can clear the issue by saving the game, and then reloading the save that I just made. However, I've noticed when I do this that the security classification is usually changed to "minimum" when I re-load the game, even if that is not what it was at the time of the save.

Maybe the problem is not the regular and padded cells but lack of solitary cells? CI prisoners require padded solitaries and normal prisoners a normal, non padded ones.
 
I don't think that's it. I have empty solitary and padded solitary cells. When I find the individual who shows unable to be assigned a cell, they are most commonly located in a solitary (non-padded) cell with no time remaining on their solitary sentence.

The act of saving and reloading a save shouldn't fix a problem if there really is a shortage of cells of some classification, but it usually does resolve the problem.

The individuals who show this problem are almost always identity thieves who have managed to pick up most or all of the various attributes. Prior to the save/reload, they are usually classified as Max or SuperMax, but after the save/reload cycle, some of them get classified as minimum, which doesn't usually go very well...
 
Go to reports book and Policy (iirc) and disable cell quality, should be somewhere on the bottom.

Btw. just a fyi, Identity thieves come from the mutator option. These extra reps are unbalanced as flip and usually ruin the game. I'd recommend you to not turn this option if you wanna have a decent prison.
