Have seen this multiple times.
Set up an alliance, and if your ally ends up paying tribute to someone else (they were also allies at the time, though not sure this is a prerequisite), the "Break Alliance" (or whatever) button is greyed out. Permanently.
Neither of us are at war so I should be able to break it.
Only way to get out of this was to declare war on the one receiving tribute, at which point I ofcourse ended up at war with both of them.
Example:
1. Sparta allies with Aechean League.
2. Aechean League pays tribute to and allies with Macedonia.
3. Sparta is now unable to cancel alliance with Aechean League (unless declaring war on Macedonia).
