When saving a game and loading a save file the ruler's appearance is reset to a default. The appearance it changes to is always the same based on phenotype.This happens to my custom race but also to the pre-made ones (Tested with the human factions and the Reptilian one).

The only DLC I have is the "Creatures of the Void" portrait pack and this bug is reproducible with or without it enabled.

How to reproduce:
Start a game.
Make a note of the appearance of the ruler.
Save the game.
Load the save file
The ruler will now have changed appearance.
