Does anyone know if a vassal needs to be included in your alliance in order for it to fight a war on your side or does it join automatically ? My assumption so far is that it does need to be included and that as a vassal it's almost a certainty that it will join the fight if asked. Is this correct ? Thanks in advance :)

Your vassal will only join you if
1) It is part of the same alliance
2) You ask it to

Military Alliance last about 10 years on average but Vassalization last until the 1800s or beyond the scope of the game. I thought someone had posted that there is a good chance your vassal will stop being your vassal if relations drops to below +140, but someone mentioned on another thread that he had a vassal with -200 relations. Anyway, I had raised this question before on another thread and it seems to be a quirk in the game. You would assume that your vassal is automatically part of your alliance, but that is not the case.
 
What's worse is when your vassal is a part of another alliance. No hope at all for getting them on your side in a fight. The worst possible case is a vassal joining what turns out to be an enemy alliance.

Yes, its very possible to have vassals with really bad Rel. I think these are primarily historical or preset vassals. Playing Austria in the IGC, Bohemia is a vassal but with -200 rel. I think it reflects a semi-forced vassalization. I'm not sure if these pre-set historical vassals break relations. Bohemia never broke with me, even with decades of -200.

Vassals you create are far more likely, if I'm right, to break vassalization.

But who really knows? (Not rhetorical!) I've seen Naples break vassalage of Spain, Brittany too. :)
