Why do vassals with 100 % loyalty, no rebellious trait and good relations keep breaking away? This problem seems to plague certain titles no matter who i give those titles they almost immediately break away. For example after taking away title of Duke of Paphlagonia from rebel and giving it to someone with 100 % loyalty within month he also rebelled. Other one is count of Tir Connail i give title to loyal character he breaks away. Few years later he wants to become my vassal again, soon after he breaks away again despite having 100 % loyalty and no rebellious trait.
 
Do you have any mods installed? Some mods include events that make vassals outside your kingdom/duchy declare independence.
 
I remember something like giving title during war will make vassal break free after the war ends.

As far as I know this is fixed in both CK and DV, so shouldn't be a problem. Might be wrong about CK though.

Not fixed in either for some cases.
It will occur in the following scenario:

You're fighting someone with title AAAA who has a vassal BBBB.
You first take title BBBB and either make him your vassal, or give BBBB to a new vassal of yours;
then you destroy (not take) AAAA, so it disappears.
In the game files, BBBB is still marked as a vassal of AAAA until the war ends, but since AAAA is destroyed, BBBB becomes independent.

To avoid this, do not make enemy vassals your vassals if you cannot grab their liege's title, and do not re-distribute conquered titles until the war is over.

Thanks for replies. Would it help if i let go provinces i got trough crusading and focused to provinces near Brittany my original Kingdom?

I'm not sure if it matters. Try to get kingdom titles for your respective provinces if at all possible, I believe this stops the independence events from firing (I could be wrong, though).
 
I observe that excommunicated vassals seem to break away from their liege quite often. I'm not sure if this is part of the event-mechanic, or a bug.

But whenever I spot an ex-com count by my border, I quiver with anticipation :p
 
A simple way I have found to keep vassals breaking away from being a problem is to always retain the title that they are dependant on. IE. if you take a Duchy, claim the title then only give out the Count titles until you can become King of the area. Note - this doesn't keep people from "breaking away" but I have yet to see a loyal Count or Duke not simply repledge a few moments later (which seems broken, but then so does the random breaking away so works as a wash.)

Note: I'm not running D.V. - just the latest patch for basic CK.
