I guess having the ability to visit your wife's chamber would be unbalanced since you could just spam it to preduce an heir.

I was under the impression that the wife has a chance to reject your request. Of course, you can do an Aerys II on the wife, but there should be consequences for that.

About this, I was surprised to note that your concubines can spurn you (Interaction, Visit Chambers) (pirate, captured Arianne Martell herself, concubine)
 
Concubines are not prisoners, they can refuse you all they want.

I could use that seeing as both me (current character) and my wife are in the same county, both in love, my fertility 85% her's 60%, and I only have 1 child (girl, imbecile, ugly, weak), 8 years old.

EDIT: I think my wife might of cheated since current character is genius and strong and wife is strong and pretty. Maybe it's because the wife is a Targaryian.
 
I've noticed that unmarried characters never reject you, or at least it's just really rare. Maybe it was because they really liked me too.
 
Having couple young ladies with good traits like strong, genius or quick and who are unmarried in court is useful to spread the seed. When it starts to seem like no male heir is coming from wife just legalize them bastards. It might sometimes take some wooing like sending gift and or multiple rap... seduction tries to get the "oven" working.

Incidentally I have never have had issues having male heirs and the occasional hosts trying to gain throne or nobles trying to install claimant are usually easy to suppress.
