I've run into some troubles with this. At first it seemed like only Slaver's Bay countries (or culture/religion, whatever) could do this, as when playing as someone in Lys I could not do any personal slave breeding.

But when I played as Qohor (misspelling? It's the Goat believers anyhow) and changed slavery laws I could, but a few years later I couldn't any more.

Does anyone know the specifics on this?
 
Nope, only being the same county mattered. Of course, if she's not a slave she has to be a lover as well, yes.

If I understand you correctly, I think you are wrong. Do you mean that you need a woman to already be your lover to suceed in the visit chamber interaction? Because I somintes "give a good tumble" to some courtiers while having no lover at all

He is wrong about that; I believe it's dependent on traits. I once played as a High Lord in the Bleeding Years start, was appointed to the council, and had a child with the King's wife and all of his son's wives.

That's an epic feat indeed! I wish I had the formula (the exact trait and modifiers that are weighted for the ai to allow you in the chambers)

I was once playing as Roose Bolton, was master of whisperes of the North. During a feast I visited the chambers of Ned Stark's wife, Ned's opinion of me subsequently went up. I've always suspected that he was watching from in the cupboard.

Nice. If only the King of the Reach had been so understanding. :D When his wife revealed the affair, he wasn't none too happy. Didn't fire me from the council though, so I kept visiting her chambers. I believe she had one or two kids that the King thought were his.

Was the wife Cat or someone else?
 
Check and make sure you're both located in the same county.

You can also go to any county with an army you are leading and visit the women there to impregnate them ^_^.
