There is a mommy bishop and a daddy bishop and they love each other...right? This is my first time playing an Orthodox dynasty. I know in Catholic lands bishops will be chosen from the court of the papal controller or are randomly generated characters of the same ethnicity as the ruler. How does it work amongst the schismatics?

Do you mean bishops as in the leader of a bishopric, or do you mean diocese bishops?
Enf91's answer is correct if you mean the first type, but I think you ment diocese bishops. In that case, you have to train your own or wait for one to randomly show up in your court (any male with an ecclesiastical education can be your diocese bishop).

Bishoprics (and ArchBishops) have their own special inheritance laws which aren't represented in the game, so they'll show up as a (fake) Elective.
Even tho it might show the Heirs of a Bishopric are the highest Martial rating of his courtiers, the new heir could be a randomly generated character, a courtier from the PC's Court (possibly only if the PC is the human player) or even from the Popes court. They must (will) be unmarried-adult-males with Ecclesiastical education (if they are pulled from other Courts).
From what I've read, the Religious Laws don't truly work as the pop-ups show, when it comes to the % chance of where new Bishops come from.
PS - The only Religious Court that will show the true Heirs will be the Pope.

Edited again because I utterly failed at reading ALL of your question. I'll start up the game and do a few F-12's on Orthodox to see, but I'd imagine they'd all be newly created characters.
Okay, ran a few tests on Orthodox Bishoprics and if there isn't a qualified male already in the Court, then surprisingly, a Country Cousin (of course - with the proper training) of the prior Bishop is generated as the new ruler.

The OP's question brought up something that I never thought to ask. What makes the AI create new Bishoprics? At the rate the AI revokes titles and captures provinces, most Bishoprics (and Arch-Bishoprics) should disappear over time, but there's always plenty out there.
My wild guess was that if the AI doesn't have any spare adult sons and he needs to create a new Count (demesne limit reached), he randomly picks a courtier in his court (mother, underage child, pot-scrubber). If that random courtier is an adult/male/unmarried with finished Ecclesiastical education, he automatically makes it a Bishopric instead of a regular Count? Or does the game say 'We need a certain % of these types', or this province is always a Bishopric - IE if the AI annexes it, the game makes him make it a Bishopric again? Anyone with Theories or Knowledge out there?

There is a piety bonus (iirc) for making bishoprics, so maybe the AI is programmed to attain a minimum piety score?
