I've noticed whenever the War of the Five Kings settles down the Iron Isles is always independent and the King or Queen of the Iron Throne doesn't have a CB in order to bring them back under the Iron Throne title.

The exception being Dany with her Dragon conquest CB...

It's been this way for long time and I'm wondering if there is a real reason for it or just something forgotten?

Maybe because they don't normally belong to the seven kingdoms. Wouldn't it be "Eight Kingdoms" if they were part of it?
I think you are misunderstanding the concept of the Seven Kingdoms...
The Seven Kingdoms is the seven independent Kingdoms at the time of Aegon's Conquest: The North, The Iron Isles, The Vale, The Rock, The Reach and Dorne. The Riverlands is not counted as one of the Seven Kingdoms.

So does the Iron Islands. They were one as Kingdom of the Isles and Rivers.

There should indeed be a dejure CB, if it's missing it is a bug. Could you please upload a save from when it was missing?

Hmm, so would it have been referred to as the Six Kingdoms before the conquest/integration of Dorne?

Reminds me of how it is practically impossible to fail to conquer Dorne at any point with the other Six Kingdoms behind you...

Aegon conquered Dorne in 4AC, he left Sunspear and they immediately rebelled. Killing the Tyrell he placed in charge (Harlen Tyrell the steward of the Gardeners). Guerrilla actions were the norm on the Reach troops there until 10AC when Rhaenys this time organized a second invasion alongside Orys Baratheon. This didn't succeed, Rhaenys and Meraxes died, while Orys lost his hand to a Wyl.

This is why he was called King of the seven Kingdoms, Aegon took Dorne, he just couldn't hold it. Or he could have held it, yet after the death of one of the three dragons and perhaps murmurs of rebellions elsewhere he scraped the plan and consolidated what he had.
