Playing as count if single county.
Married to countess of neighbouring county, within the same duchy.

Wife's title is revoked by the duke.
I start a faction to give her county title back- we win the war against our duke and she's a countess again.

Few months later, the duke has simply revoked her title again.

Could someone explain why this would happen, and if this is how it's supposed to work? It seems broken but I don't know if I'm missing something.

Okay thanks.
Should there not be something stopping this from happening? If he'd lost a normal war over a title, there would be a truce to stop immediately trying to take it again.
Yet the war seems meaningless here.

The issue is that landless rulers cannot have truces and cannot participate in wars. So the duke have a truce with you and other faction members, but since the wife was unlanded and did not technically participate in the war, there is no truce for her.

But for Great Holy Wars, the new Crusader King/Queen gets always an Truce with every single Character in the Region and the Members of the Great Holy War, to prevent this from happening, that they lose their Title instantly, if the Devs would have copy this to the Faction Wars this would prevent this too.
