Well, I've got some time off, so I took to modding a bit. The result of two days of suffering and running the game dozens of times is my very own AGOT submod to flesh out the piratery a bit. It's been tested mostly through console commands, mind you, and at this point it is fairly rudimentary.

Features:
- Replaced pirate piety with Authority. This reflects your standing among the pirate masses, same as Prestige does among the high lords.
- Piratey traits and good leadership qualities give high authority gain (e.g. fighter traits, good military education, ambition) and unpiratey traits have a high penalty (i.e. content, humble, wrong or bad education), all for rulers only (some traits and education outcomes are neutral i.e. they don't give you any gain or loss), handled by character modifiers (because I have no idea how else to handle it and redoing the trait system isn't an option)
- Baseline piety gain for independent rulers per rank per month: 2 for counts, 3 dukes, 4 kings, 5 emperors
- Negated piety gain for zealous/cynical, lustful and hedonist traits for all pirates
- Independent ruler pirates with high authority have access to 3 once in a lifetime decisions to call a pirate council to basically crowdfund their explosive start - one is for a 100% interest lifetime loan, another for some light infantry and the last for ships, with more options for the ambitious and smoothtalkers

This is all to reflect the here-and-now piratey frame of mind where the focus is less on managing a dynasty, expecting loyalty and position due to noble blood and more on managing a pirate, commanding obedience because of their qualities, so while a pirate king who is notorious for his desire to plunder and skill in battle might get followers and loans easily, another who is content with sitting on his rock is going to get little more than passing glances.

Planned:
- Rewarding success in wars (conquest/taking noble prisoners/concubines)
- Punishing failure (losing wars)
- Events to burn off excess authority in exchange for minor benefits
- Penalties for negative authority (I'm clueless here)
- Hiding character modifiers for traits or moving them elsewhere
- Icons for decisions
- Branching events for councils
- Events for forgiving low tyranny/dishonorable actions a few times (because you're a pirate, not a saint)
- A way to take a ruler's entire treasury by looting his capital province
- A way of negotiating with a ruler when winning strongly to leave his lands be and leave (by demanding 1, 2 or 3 of the following: his wife, his daughter(s), his money) and a way to offer the same to whoever manages to beat you

Installation: Paste the contents of the yarr folder over (documents directory etc.)/mod/A Game of Thrones.
There should be only one conflict, which is the religion file, which I had to modify so that pirate piety is displayed as authority.

I should update at least weekly, but then again I have an EU3 mod to finish on my to do list since 2011.
 
I don't think you really need penalties for low authority, not having the benefits of high authority should be enough.
