@Max_Killjoy has the right of it - the Command Consoles grant 3 extra "hit points" to the pilot. This is meant to represent the 2nd passenger in the 'Mech (with their 3 HP).
If you select the enemy 'Mech, you will see an icon indicating they are gaining bonuses.
Yup, but the combat messages are not saying injury ignored for 3 hits if you knock them down, even though they are

Not sure why that happens. Might be something with the AI using the mods. I may try modding a standard cockpit mod onto a test 'Mech and see what happens with head hits. If that is the case, then it is in the game itself that way - as the designers never expected us to do something crazy like that

I have no idea how BTML handles it.
I have a home-brew mod to add some mechs and put "break.my.game" file in "BATTLETECH\Mods" folder not inside my mod's folder and it worked as intended (my new mechs show up as OpFor in the campaign).
It's possible that BTML is very sophisticated and if the break.my.game file is in an individual mod's folder it will only add those items to allow selective additions to the database.
BTML might look for "break.my.game" in the top level of "BATTLETECH\Mods" and treats that as direction to add all new elements in any mod.
It's also possible BTML looks for "break.my.game" file anywhere in Mods or the sub-directories and adds everything it adds to the manifest csv to the database.

break.my.game is just a toggle that needs to be in the root folder
it can even be deleted after it was used once everything was added

I had multiple break.my.game files before. The only way I got it to work was remove the ones inside the individual mod folders and just leave one of the break.my.game file on the root of the Mods folder.
