Diseases were very impactful on the development of cities and even empires and greatly influenced social interaction and thus trade, as is most obvious to most people since at least last year.
Plague-events are already implemented for example in EU4 with options of quarantine or letting it be with short-term vs. long-term benefits,
but we also need e.g. Leprosy-events here in Imperator Rome.

At least I think that they should be there for flavour. What's important though is that the player is able to react to the events in an impactful way in contrast to having it as a "comet event" with no control whatsever, just punishing the player for just playing at random. Not good for the economy ;)

I think that it should be implemented with a trade rework and be directly influenced by this

The immigration (intended as pop moving) and the army's movements too could spread the disease.
Also overpopulated cities and metropolis should be the epicentre of diseases diffusion and to counter it I would introduce a sanity mechanic and so more buildings related to it ( like the system in Attila Total War)

I was merely suggesting a simple event chain, you guys however are developing a whole new game-mechanic that would have to come with its own patch. :D
I like the added ideas though. :)
