Was playing as the Sikh empire, stole all of India from the UK. Clicked the button to unite India, only got claims on all my puppets.

OK, I did some testing using the Debug Mode and it seems that having the BIC as a subject is what causes the bug to happen.

Start as Punjab.
Annex BIC.
Form India. All minors are annexed.

Start as Punjab.
Ctrl+click every indian state (leaving BIC alive in Indochina).
Wait 1 day. Release all Indian minors as puppets.
Form India. All minors are annexed.

Start as Punjab.
Demand recognition from Russia, change tags and accept.
Transfer BIC from UK, change tags and accept.
Reduce BIC autonomy, change tags and accept.
Form India. Nothing is annexed.

Same as above, but ctrl+click the Indian states of BIC (leaving them alive in Indochina).
Give BIC independence.
Form India. Nothing is annexed.

Same as above, but instead of reducing autonomy, annex BIC using the console.
Form India. Nothing is annexed.

It appears that if you subjugate the BIC at some point (even if you later annex or free them), the game keeps you from annexing your rightful Indian clay when you create the tag.
 
After some extensive testing, I have found that the bug is not specific to India.

The basic problem is that when forming a country, the game will not absorb and/or take states from countries that are of a different status than the overlord. Recognized powers will only integrate recognized powers, and the same goes for unrecognized powers. The Raj (BIC tag), however, will not be integrated at all.

In the save files below, there's a pair of files for each scenario:

Scenario 1: Sikh Empire 20-21 - Unrecognized and Recognized Punjab trying to form India. Unrecognized PUN eats the princes, does not eat BIC. Unrecognized PUN eats nothing.

Scenario 2: Sikh Empire 10-11 - Same as above, Belgium replaces the BIC. Unrecognized works the same as above. Recognized eats BEL but not the princes.

Scenario 3: Sindh - With only unrecognized powers, Sindh forms India. If Sindh is recognized, no land is gained.

Scenario 4: Egypt - Unrecognized Egypt eats the vassals except Assyria (ASS is recognized?!). Recognized Egypt eats only the parts of Assyria that fall within Arabia.

The script involved in country formation is: CountryFormation.FormCountry() but I haven't been able to find it for editing (I'm assuming it's compiled in the game files).

Hope this helps sort the issue.

Excellent sleuthing!

For BIC, it's defined country type is "colonial", which appears as recognized in game, but is used for much of the Americas as well as East Indies and Australia, hence why it wasn't absorbed.

You are right, I was able to take states Ecuador and Miranda from the BIC when forming Gran Colombia with the BIC as a subject.

Ok, so the final verdict is that only nations of the same country type get absorbed when forming countries.

If it is the exactly same issue adding the save here is actually fine.
Setting this one to "Need Info" for the save.

Here a 1.8.6 save game with the issue when Egypt forms Arabia where no subject get annexed.

To corroborate bobthedog theory, I made a test forcing completion of "je_earn_recognition" on one puppet before forming Arabia, and this time it was annexed.

Happened for me again on 1.9.2. will all the non-cosmetic DLC activated. I really don't know how much help my Ironman save would be and I really don't feel like figuring out how to mount my USB drive to my Linux system right now to transfer the save so you'll have to do with just believing me. For some reason my USB isn't showing up in dev. What you probably want is a save just before you click the form nation button I would guess. But since recognition has become a lot easier to get before forming Arabia, I've been experiencing this bug every time I play Egypt for probably the last year and a half.

Good work on 1.9. It was a lot of fun. Can't wait to sink the British Navy to the bottom of the ocean permanently in a later expansion.

Only relevant save I have is both (extremely lightly) modded and post-unification so like above not sure how much help it could be, but can also confirm its still an issue in 1.9.2.
 
I've attached two saves in which Spain can immediately form Iberia. In one, it annexes both Portugal and the Philippines, in the other only Portugal.
