Playing as King of Poland, I had granted the county of Plock to my mother. In due course of time, she died of old age, and the county reverted to me as her heir. I decided to appoint one of my courtiers as the new Count of Plock, but something odd happened--he stayed in my court. His portrait has the trim around it showing him to be a count, and it says he is Count of Plock, but Plock is apparantly still in my demense. It shows up on the map as part of my demense, there is no shield there (even the empty shield that sometimes shows up on the map in various counties), and I can build improvements there. On the other hand, Plock isn't being counted against my demense limit. Also, I'm not able to appoint someone else as count. Saving and reloading didn't fix the problem (though at least the anomaly didn't cause the game to crash, at least not yet). The supposed Count is an old guy, and I'm hoping that he'll die soon and the problem will correct itself, but I can't figure out how it happend. I've appointed other counts in the same game with no problem.

Did you wait a day to grant the title anew after you inherited it? There is a problem already in vanilla CK if you don't, namely the one you describe.
