It has been a disturb thing to modders, "I've my OS re-installed and I can no longer update my mods", "I buy a new PC, how can I update my mod from it?"
When it comes to me, I did some attempts to figure it out. No luck then.
But new PC eventually comes(uh!) and I have to play and mod on it. I finally manage to re-sync all of my mods on Steam Workshop from new PC.

I'll show you how to do that, step-by-step.
  1. (Recommended) Unsubscribe your mod and delete local downloaded files, sometimes I subscribe my own mod for +1 subscriber testing if the mod is correctly packed or not and no dependencies errors. Before you re-sync your target mod, unsubscribe it and delete both great_mod_name.zip and great_mod_name.mod.mod. Leaving the original great_mod_name.mod and /great_mod_name folder only.
  2. Launch CK2 from Steam, not double click on CK2game.exe.
  3. (*) Steam must be active. Make sure you are not under offline mode. Using tab+shift overlay window to have a quick look.
  4. Content Settings>MOD>find great_mod_name and press Manage (active or not makes
    no difference)
  5. Publish. Don't be afraid you'll have your mod overridden. It could not be even worse.
  6. Now CK2 should A) CTD, as usual; B) nothing happened, like a dumb; C) A new item published(!?!?)
  7. Quit CK2(and delete that new item if published), re-launch CK2 from Steam(step 2. 3. 4.)
  8. Now the button should display as "Update" with Description. Congrats.
It was actually an old tips but unproven(at least for me then). I'm glad to confirm this is a valid solution for people who troubled by this problem.
