Please explain your issue is in as much detail as possible.
I started a new campaign. I selected Ironman as the sole change to the default settings. When I attempted to save and quit game I discovered that the only option available to be selected in the game menu was 'Settings'. No save (well Ironman, I know) but also no 'Quit to main menu' nor 'Quit to desktop'.

There was no save made automatically (if there was meant to be one) and having to kill the process in Task Manager meant no save on exit.

If the game crashed, please attach the crash file with the time stamp closest to the crash.

Seem to have been able to get the bug again. As you can see, not in combat, between missions. Same shows during combat as out of it. To confirm, game files have been verified (nothing downloaded). No mods. Something which probably isn't related but did strike me during the past couple of times it has happened is that the prologue's cut scene where the rebellion is announced is absolutely silent while the cutscene and subtitles play. Otherwise, not seeing anything unusual in game behaviour beyond not being able to quit the game.

Just additionally, I have no desire to see whether it persists for a further 6 hours of gameplay as it did when I made initial report.

And just to show Ironman itself can behave normally, this is from a campaign started with the Prologue turned off and Ironman turned on in the starting settings. Screenshot taken at exact same gamepoint other for not doing the prologue story mission.
 
Ok, so a clean reinstall to C: seems to have fixed the save game issue (for now...). The prologue speech about the rebellion is still silent though but win some, lose some. Still baffled as to why Ironman would break with Prologue but not if Prologue were skipped.
