I did the 1st HM flashpoint, but then delayed doing the 2nd one and now it's disappeared from the navigation map. Game is post-career and there has been no sign of it returning.

I am having the same issue. I did the first flashpoint and IIRC, got the event that unlocked the next flashpoint, but continued on with my career, and now it's gone. Some have said the flashpoint actually does have a timeout, even though it doesn't display one, and no means to respawn. There's been no comment on this bug for a while, so I wanted to point out that it's still an issue.

No update, and the last patch seems to have had no effect. I think our only option is to start a new campaign/career.

Just wanted to confirm I also have this issue in my game. Bug is exactly as described in the OP - I did the first mission and delayed doing the second, and now it's gone. Can confirm it's not just me not seeing the Flashpoint due to the Black Market symbol - I went to Mantharaka where it should be and it's just not there anymore.

Hi there!

Just a small message for people who would still be encountering this issue: I published a mod on Nexus as a workaround to it. It works by allowing (a duplicate of) the event responsible for the spawn of the next campaign step to pop again.
