I think very rarely indirect LRM fire can happen without LOS, I have only had it once in something like 500 hours play although in my case it was a turret that wierdly appeared to be getting vision from buidlings (although it may have been another turret I destroyed by walking into range of) once I completed that mission, I have not had it happen again, that was probably 50 or 60 hours played ago now.

I can confirm this bug. If you save partway through a mission and reload the enemy retain vision on your mechs and continue to fire at them until you retreat them a certain distance and then return to the fight. Given the AI's habit of continuing to pummel your most damaged mech even at the expense of fighting stuff right in their faces... can be quite annoying. I've basically stopped using the save option except at the very beginning of a mission.

Hmmm I will keep an eye out for whether it is related to quitting and coming back, I have only had this once and I very well may have quit mid mission and come back (on Ironman mode).

I wasn't on ironman mode. But I do know that saving mid mission during combat can be funky. Nowadays I only save when not in active combat if I can help it... or if the combat is really close quarters, because otherwise breaking contact can be near impossible.
