Ok down and dirty. This is repeatable, and can be confirmed with about 20 minutes of game play. If you DoW a nation and start taking its home territories and grant a title on one of them before absorping them fully. The vassal thus created will immediatly become independent with the defeat of the last enemy province.
Example: Playing as Byzantium I DoW the Pechang, after beating them at birland I grant a courtier the title count of birland, note they are my vassal at this point. Upone defeating the last pechang provience when the war ends the new count of birland is independent.
This isnt that big a problem unless you need to shed a few provinces to vassals to keep from getting crushed by demense penalties. However, if you are in a large scale war then you can have whole swathes of your vassals vassals become independent if your vassals are granting titles as they go.
