Please explain your issue is in as much detail as possible.
In certain circumstances, it is possible to have a vassal who is revolting against their liege die and the title become inherited by someone who shouldn't be able to hold it. In this case, one of my sons had become the vassal of another son & revolted, only to die of an illness. I didn't actually notice any ill effects of this for almost a year, until I went to declare a war and discovered I was no longer able to declare wars/destroy titles/etc. due to holding a temporary title.
Steps to reproduce the issue.
Fabricate a scenario in which one of your sons revolts against his liege to become a vassal directly below you without children (making you his inheritor). When he dies, you will inherit the titular Revolt title, but the revolt war will become invalid (since you'd be an attacker against your vassal). You'll end up stuck with a Revolt Title that shouldn't exist, which you're unable to remove.
Save in which this occured was modded, but none of the mods affect title inheritance/revolts/etc - it's just an assortment of uncappers for our Immortal Dragonkings game. ( https://steamcommunity.com/sharedfiles/filedetails/?id=1638884931 )
