Please explain your issue is in as much detail as possible.
In my current game, I have handed out many counties without creating vassals for the extra baronies (or by handing counties to the vassals I create from mosques), meaning many of my count-tier vassals have multiple holdings per county. But they are hanging on to all these baronies even when far over their demesne limit, instead of creating barony-tier vassals of their own.

I had something like this happen too. In my game, it was a mayor who I gave three counties to, along with lower titles. He held on to everything even though he was 3 over his demesne limit. Later I gave him the duchy once I got a kingdom of my own and he finally gave away those holdings.
