Please explain your issue is in as much detail as possible.
In a Random World, every patrician in the same merchant republic is given the same dynasty name. It makes it very hard to tell who owns what trade post, and so on. The names used are also typically the "of Placename" form, which just looks bad when they're displayed as Patrician of of Placemname.

This doesn't happen for every generated Merchant Republic, and seems to be related to culture in some way. For example, I have seen both a Bodpa and a Kirghiz MR where the patrician houses got distinct names. Randomizing culture names doesn't solve this, though.

Steps to reproduce the issue.
Generate a Random World with the option for Merchant Republics active. I have seen this happening with the default options set, so you shouldn't need to change anything there. You don't even need to click Play, because the patricians are all listed as vassals on the character select screen.

This is unfixed as of patch 3.0.1. The added ability to change any title with the Customizer DLC does alleviate it somewhat, and if that's the best that can be done, I'll accept it.
 
This has not yet been fixed in patch 3.1. In the many months since I first discovered this bug, I have developed a theory about why it occurs. Some cultures are scripted to have dynasty names generated based on the first name of the dynasty founder, and some are scripted not to do this. The bug only occurs in the former case. I have observed that the one dynasty name shared among all patricians is named for the capital city of the Merchant Republic. So I believe the bug is caused by the order in which things are generated. It looks like the generator assigns a single city to be the capital of a Merchant Republic, and then the patricians are generated before any other land is assigned to the MR. Since their culture forbids their dynasties from being named based on their first name, the game has only one choice of what to name them after: the name of that one capital city.
