Please explain your issue is in as much detail as possible.
On the side tabs of the game, I have the usual listing of demesne holdings, great works, trade posts, etc. But on the first of those, the demesne list, at the end of all holding main title (the castle, since I'm on a imperial game) a weird extra shield appears followed with a "..." instead of the holding's name. The CoA of the shield seems to be the Kingdom of Sweden, that currently has no holder. Nobody in my realm, that it's far away from nothern Europe has a claim or something close to it, mainly because it has never been created, so it must be a bug. The Kingdom of Norway exists and has currently a holder. There is a petty kingdom of Sweden, but it's a duchy-level title. My current ruler dying and being replaced by his heir didn't fix the holding listing,

Hmm. It seems to appear each time that the holder has a county belonging to another duchy. In the above example: Antioch and Alexandria. Still not sure if it's intended, or a bug.

Nope. It's not the case, it seems to happen once the number of counties in the demesne exceeds 13.

Ok, here it is. The problem exists no matter which country you're playing as.
