When you get longlasting symptom like chest pains, which lasts for many years and then get a disease like cancer, plague, consumption etc. and ask your court physician for treatment via decision, he will not treat your disease. Instead he will only treat your symptom.
Steps to reproduce the issue.
Get one of those longlasting symptoms like chest pains and then catch another, proper disease like consumption. Then ask your court physician for treatment and he will only treat the symptom, often saying "hmm this may be developing into plague or cancer", which made me believe this is indeed not intended to work this way.
