One line summary of your issue
Eniatic clans can leave hierocratic reformed pagan church headless
Game Version
ONDO
What expansions do you have installed?
All of the above

No
Please explain your issue is in as much detail as possible.
When I reformed the Slavic church with the Eniatic Clans doctrine and Hierocratic church leadership, the male priest who lead the Slavic holy site temple in Kiev automatically was granted the title 'Slavic Church' and thereby became the Slavic pope.
He ran the church for a while, maybe a year or two, but after a while he became a feudal leader (I'm not totally clear on why this happened) and dropped the Slavic church title, leaving the church with no pope. Now, I have no way to reinstate the title and the reformed Slavic church is left without the benefits given by a head of religion.
Steps to reproduce the issue.
Reform a pagan church with the Eniatic Clans doctrine and Hierocratic leadership, making sure that all holy sites controlled by the religion are held by men, preferably without families (I think it might be important that when the temple title goes to 'eniatic open', there is no valid heir to the holy site/church leadership title)
It might also be necessary for there to be a non-player ruler of the county that contains the holy site. I suspect it might have been that character's conversion to reformed slavic that caused his vassal the slavic pope to switch to feudal government, although I am not sure. (I'm not sure what else changed at the time that caused the slavic pope to drop his religious title)
