Please explain your issue is in as much detail as possible.
I have been going to war and being unable to siege the target county because i am not hostile to the OCCUPANT*. So, I can't end the war unless I can first figure a way to become hostile to the occupant or I won't get the counties. *When i say occupant I mean the one currently in control of the holding not the OWNER of the county.

Wars are ending without clearing the occupants. In other words Scotland just invaded Noregr revolt (from now on i will just say NR for Noregr Revolt) via holy war and won but when the war ended, scotland is still occupying one of NR's counties that was outside its target duchy. The county where scotland is occupying (but not at war) is one of the target counties for my war. I would need to become hostile to scotland to occupy that county. But scotland is my vassal. I also occupied one of the counties that was Scotland's target so it didn't get that county when it ended its war. (I took it accidentally)

Then, another county in my target area for my war is occupied by Jarldom of Viken but Jarldom of Viken is not directly at war with NR. Viken is Noregr's vassal (not NR) so it is hostile to me and to NR because of the war against his liege but I am not hostile to Viken so I can't assault the holdings occupied by Viken.

This is not the first war this has happened. This game has been really chaotic and I think this is the reason why. Let me explain. Nearly everything is controlled by a 6 empires but the empires have speckled counties through out one another. The map is a real mess I have never seen a map like this in all of the times i have played ck2. There have also been an unusual amount of stray holdings. I could easily see how this could cause stray holdings.

If I end my war now (which I suppose I have to what else can i do), I will only take 2 of the 4 target counties from my holy war. The only way i can think of would be to declare war on noregr to become hostile to viken but I can't do that because Noregr is protected by defensive pacts. Then I would also need to provoke my vassal into war with me to become hostile to my vassal and clear its occupation of my target county.

The game is broken.

Let me reiterate one point. Viken, the vassal of Noregr is hostile to me because I am at war with his Liege's revolt. It shows him as hostile to me in his diplomatic window. But I am not hostile to him. I suspect Viken declared his own war vs noregr revolt, the war ended, but the occupation didn't clear the same way scotland's occupation didn't clear when Scotland won its war.

Also, the county where I took a holding that was in Scotland's target area on accident, I only took the castle, scotland got the other holdings. Do you see what i mean by "this would explain why there are so many stray holdings"?

There is another problem i noticed that could be related. I have fought 2 holy wars this game and in each it was very difficult to maintain control of sieges. Even if I managed to be siege leader when a holding fell, I wasn't getting warscore credit for it. The same thing was happening in battles. I move in better generals and my king and hold all 3 flanks then some 10 martial mayor who came later takes control of the flank. Then, again, I don't get participation credit. In a holy war vs Croatia, I did all of the sieging except 1 county and a lot of, if not most of the fighting but got only 10% participation. I definitely had the most particating troops by far. Some duchy had 33% participation if I remember correctly. Then at 100 warscore the target Croatia decided to become catholic = cassus belli no longer valid.

I say it may be related because Scotland, my vassal managed to steal control of a few sieges from me while we were both at war with Noregr Revolt. When they ended their war, those counties cleared occupation as they should. It is the county they sieged on their own that didn't clear. When they stole the sieges, it appeared on the screen I was in control of the siege but when the siege was over it was their flag hanging over the castle.

In that holy war for croatia I didn't get participation credit for 1 siege where I was the only country sieging the county. Then, on top of that I think when my ruler died in battle my participation reset to 0.

One last issue. Just prior to my holy war with croatia I attacked or was attacked by another member of my crusade team. It was my vassal. I think he tried to plot my assasination and I attempted to capture him but failed. Once all of the crusader armies popped up blue I couldn't see my vassal's armies that were both enemy and ally. His armies weren't red they became blue. Lucky for me the war was almost over but it could have been a problem. Our armies still fought each other despite his army appearing blue. Luckily I knew where it was.

SUMMARY
1. holdings need to clear occupants when a war is over. They aren't always clearing now.
2. If I am at war with someone who is at war with others, I need to at least be able to become hostile to my enemy's enemies in order to complete my war. (in the case of Viken, I should have already been hostile but I wasn't.
3. Something is wrong with the way crusade participation is being tallied. In past games I have played I mostly sieged for warscore/participation and fought only when i needed to but sieging recently didn't seem to be effecting participation.
4. If I am an enemy of someone who is also my ally in another war he should appear to be enemy not ally.

I have no mods

Steps to reproduce the issue.
I'm not sure if I could reproduce the #1 & #2 issue but it is reproducing itself.

#1 & #2 - Two nations go to holy war with a revolt of a large nation. (I don't think it needs to be a revolt though) (I don't think it needs to be a holy war though catholic-slavic-norse have been holy warring each other a lot)
Each nation occupies 1 county from the target area of the other.
Now neither can end their war and get their entire target area without finding a way to become hostile with the other and removing the other from the target area. Scotland's holding that didn't clear occupation was outside its target area. I had one holding occupied in Scotland's target area.

I don't know how to reproduce the holdings staying occupied after the war is over but that might do it.

#3 - Siege stealing and combat leader stealing was really bad in the holy war for croatia because there were so many troops in such a small place. So, making a holy war in croatia, which is small might reproduce what i saw. Then, kill the leader and see that it resets participation. (I think it had to have reset)

#4 - Start a war with a fellow crusader right before the crusade begins.

update. Viken was allied to its liege in its liege's war against the rebellion so it may have taken that holding during that war rather than it being left from a previous war as Scotland's was. (I made note of that in one of the screenshots)
 
update. After ending the war I only got 1 of the 3 target counties. The county occupied by scotland cleared when I finished my war but returned to Noregr Revolt's ownership. Scotland did not get the county I had occupied. (I didn't expect it to but I think it should have)

So i got screwed out of 2 counties
1. The ally/vassal of the liege being revolted against held one and there was nothing i could do about it
2. The county that didn't clear occupancy when my vassal finished his war

My vassal got screwed out of 1 county
1. Because I accidentally occupied it when I tried to defend it and my vassal had no way of getting it back
