Please explain your issue is in as much detail as possible.
Playing shattered world with full randoms.
At some points many kingdom was formed on the maps.
Some of them was subjugated by local nomads or "Mongols".
In most of them Nomad liege (most probably) revoked kingdoms to himself and destroyed titles. After sometimes local rules gained their independence back - but these titles are now marked inactive and can't be restored.
It thing this is somehow wrong.
It's not wrong to mark titular titles but titles with land should be recreated on some conditions.
I guess some decision to recreate titles with de-jure lands should by available to players (and AI).
Might not even on 51% but on 100% but still.
Steps to reproduce the issue.
1, create a shattered game.
2. wait for kingdoms formed by local rulers
3. wait for nomads arrived and subjugated them
4. as they revoked titles to themselves and usually destroy them, titles marked as inactive.
5. when this land would be free from nomads no one can recreate titles again.
