There is no decision to switch to the Outremer culture. It isn't there at all.
I started games in post-Crusade start dates to test it out, and I'm absolutely sure that I met the conditions (I was the King of Jerusalem at various dates between 1099 and 1187). Giving myself 10k prestige or forming the Empire of Outremer had no effect.
Steps to reproduce the issue.
Set all options to default, and play as a crusader state on an appropriate date.
