I've seen this happen on two playthroughs.
As Emperor of the ERE, one of my vassal exarchs seems to get stuck in a loop revoking one county title, issuing it to someone else, and then going back and revoking it from the new holder after only a couple weeks.
I've seen this happen with Exarch of Greece revoking c_chaldike, and Exarch of Anatolia revoking c_ikonion. While I can't check the Greek Exarch, the Exarch of Anatolia does not appear to have Paranoid or Arbitrary.
I'm also not sure how to copy the file for a cloud Ironman save, and upload it. If you can tell me how to d/l it out of the cloud, I'll upload the two saves (as long as it doesn't turn off Ironman.)

I'm not sure how to reproduce it actually. I've just observed it twice when I keep getting messages "Exarch Repetitious revokes County of Ikonion" every few weeks of game time.
