Please explain the issue you experienced in the most condensed way possible
In an older version (tested on 1.0.3), a newly-created head-of-faith would have a unique coat-of-arms, with symbolism related to the faith. Any holy orders created would also follow a pattern related to the faith.
However, in the current version (1.2.2), all new heads-of-faiths and holy orders, regardless of faith, get a generic pattern that is in no way related to the faith.
This issue has already been reported in the past (on 1.1.3 back then) but it was not addressed, possibly due to lack of evidence that the bug was introduced with a patch 1.1.x. Now there are screenshots comparing versions 1.0.3 and 1.2.2.
Please explain how to reproduce the issue
Reform a pagan faith (or create a branch of an existing faith) in a way so that a head-of-faith would exist, and observe the coat-of-arms of the head-of-faith. Create several holy orders and observe their COA.
Revert to version 1.0.3 and repeat the above steps. Observe the COA this time.
Is there anything else you think could help us identify/replicate the issue?
The problem might have been introduced with one of the patches in the series 1.1.x, but in the current version 1.2.2 it has still not been fixed.
It is expected that newly-spawned mercenary bands will have the same problem, but I have not seen a new mercenary band so far.
