Please explain the issue you experienced in the most condensed way possible
I used conquest CB for the duchy of a larger country. When enforcing my demands the window as normally said that I'm going to get all counties in the duchy belonging to defender. However I only got one county out of many (and the target duchy still belonged to original realm). What actually happens is that the enemy realm ruler gets the contested duchy from her former duke and the counts in it switch the liege to her, but all counties except one remain in original realm The title log shows that on the day of war's end the title is usurped by the losing defender ruler.
Please explain how to reproduce the issue
In the attached save file there is an ongoing war with 100% score. Enforce demands and see what happens.
