Is this a bug?

When I am at war and I have besieged and now control several provinces from several different enemies, every so often, the provinces all revert back to their original owners.

There is no message of why this happened.

Can someone explain this to me?

1) Have you responded to a call of your allies?
If so, they will have made peace and didn't give you any provinces in the Peace Resolution.
2) were they part of an alliance? If you make peace with the leader of the alliance you will only get the provinces you demand from him (if he accepts)
