I mean, I guess I understand them wanting some time off to grieve but somehow I don't think this is intended. When a councilor's wife dies they drop out but can be immediately appointed right back.

I am unable to replicate this.

At least, not by assassinating the wife of one of my councillors.

When you notice this, is it limited to natural deaths? specific council positions? specific ranks for the councillor in question (are they landed or not) ?
 
To me it happened for at least two dukes and one baron. When their wifes died they left the position but I could immidiatly choose them again. Can't say for certain that they were natural deaths as there was quite many plots going on in my kingdom and I usually don't mind some random courtier killing some other courtier. It has happened atleast to my spymaster, chancelor and marshal so it seems to be with all the positions. I would recomend testing this by letting some courtier kill advisors wife with a plot as it might have been a plot that killed those wives.
 
Hmm... okay, it seems like it's not a 100% of the time thing, since I did "lose" my chancellor due to his wife's death this time.. But the Marshal remained in his position throughout.
 
This seems to happen 100% in my MP game. I was doing a cycle of ambition: become spymaster -> plot to kill wife -> remarry for a while because the king would always re-offer it as I automatically quit when I assassinated my wife

That is such a cheesy exploit that I hope your liege has you set up a spyring in your capital and assassinate yourself.
